When initiating ACE inhibitors, it is common for blood pressure to drop dramatically. In individuals who are salt-sensitive or have underlying volume depletion, the drop in blood pressure can be more pronounced, contributing to the risk of hypotension, especially after the first dose. Monitoring blood pressure closely after starting therapy is crucial to ensure patient safety and to manage any potential adverse effects.

While blood pressure might remain unchanged or only rise after multiple doses for some patients, this does not negate the reality that many experience an immediate and significant drop when first started on an ACE inhibitor.
